District Nurses
The District Nursing Team provides nursing care to patients who require care to be provided at home. This not only involves routine visits, but also includes diabetic assessments, elderly screening, cardiac rehabilitation (Phase 2), leg ulcer assessment and management, terminal care and follow-up bereavement care. Community nursing care is provided over 24 hours, with the evening service responding to calls out of hours.
The District Nursing Team continually strives to develop the service to meet ever-changing needs. Current objectives include improving patient-held records, and chronic disease management for housebound patients.
Changes in the skill mix have demonstrated improved efficiency, teamwork and quality of care, by facilitating a District Nurse to be based at both Taymount Surgery and Scone Surgery.
