New Patients Registering with the Practice
To register permanently with the practice you should be resident within the practice area (see map below) and you'll need to complete a registration form and our patient questionnaire.
You can download both forms as editable PDFs, complete them, attach to an email, and send them to us:
You may need to download Adobe Acrobat Reader to edit and save the forms.
At the moment (due to coronavirus) we will accept completed forms without a signature.
Please email the completed forms to us at: Taymountscrips.Tayside@nhs.scot. You do not need to attend the practice to register.
Further information about registering with a GP practice is available from NHS Inform.
The Practice Boundary
The practice area is shown marked on the map below:

New Patient Appointments
When you give us your application form you will be given a new patient appointment with the Health Care Assistant and asked to complete a medical questionnaire. Patients are registered with the Practice and not an individual doctor, but you can of course see the doctor of your choice.
The Patient Charter
You will be treated as an individual and will be given courtesy and respect at all times. You will receive the most appropriate care, given by suitably qualified people, and no care or treatment will be given without your informed consent.
In return we would ask you to treat all doctors and staff with courtesy and respect. We would also ask that you try to follow the medical advice offered, and take any medication as advised.
For further information see how to work in partnership with your doctor.
