New Patients Registering with the Practice
In order to register with the practice you must be resident within the practice area (shown below) and complete a new patient registration form which should be brought into the practice. A copy of the form is also available from reception on request.
Remember to bring the required identification with your completed form - we require two forms of identification as follows:
- One form of photographic ID e.g. passport or photocard driving licence
- One proof of address e.g. a recent utility bill with your home address listed

New Patient Appointments
When you give us your application form you will be given a new patient appointment with the Health Care Assistant and asked to complete a medical questionnaire. Patients are registered with the Practice and not an individual doctor, but you can of course see the doctor of your choice.
The Patient Charter
You will be treated as an individual and will be given courtesy and respect at all times. You will receive the most appropriate care, given by suitably qualified people, and no care or treatment will be given without your informed consent.
In return we would ask you to treat all doctors and staff with courtesy and respect. We would also ask that you try to follow the medical advice offered, and take any medication as advised.
